Request for License History...
Students who enrolled in any MTO approved driving after September 28th, 2008, and need to have a paper copy of Proof of Beginner Driver Education Course Completion need to download the application form and mail it to the MTO at the address on the form. Driving schools will continue to issue the certificates to students who have enrolled prior to September 28th, 2008.

The Driver Licence History Search can only be requested by the licence holder, as it contains personal information. a separate application must be completed and signed by each applicant. The MTO charges $12.00 for the search. $18 if a certified copy of the record is required.
